According to Happy, if you wriggle out of the straps holding you in the chair at the beginning of the Call of Duty Black Ops single player game (rapid R1 R2) you get out of the chair and make your way to one of the computer terminals where you enter a code which unlock the extra maps!

being in mind if you unlock them in this way then it voids getting the trophy for hacking the terminal properly.
The official way to get them is by completing the campaign to play in the Pentagon and the correct code for Dead Ops is “DOA”. Kino De Toten is obviously unlocked from the beginning.
